What "Leaky Gut" Really Means (and What the Evidence Actually Supports)

Leaky gut is everywhere online, usually attached to a test you can buy or a powder you can take. Here is the plain, cited version of what is real, what is still unknown, and what actually helps.

If you have spent any time in wellness corners of the internet, you have met leaky gut. It gets blamed for bloating, brain fog, fatigue, stubborn weight, acne, anxiety, and just about every autoimmune condition. It usually arrives with something to sell, whether that is a $200 at-home test or a powder promising to seal your gut lining.

Here is the frustrating truth. There is something real underneath the hype, and it has been studied seriously for decades. But the version being marketed to you has run far ahead of what the research can actually support. Let's separate the two.

The part that is completely real

Your gut lining is remarkable. It is a single layer of cells, and it has a genuinely difficult job. It has to let water, vitamins, and the broken-down pieces of your food pass into your bloodstream, while keeping bacteria, waste, and half-digested food out. The cells are stitched together by protein seals called tight junctions, and those seals are not fixed. They loosen and tighten depending on what is going on in your body.

When those seals loosen more than usual, researchers call it increased intestinal permeability. That is a real, measurable phenomenon, not a fringe idea. A well-known review in the journal Gut laid out the mechanisms, the ways it is measured, and where the science stands (Camilleri 2019). So when someone tells you the gut barrier can become more permeable, they are not making that up.

The part that is not settled

Here is where the marketing and the science part ways.

Increased permeability shows up in several conditions, including celiac disease, Crohn's disease, and some cases of irritable bowel syndrome. What researchers have not established is the direction of the arrow. Does the loosened barrier cause the disease, or does the disease loosen the barrier? For most conditions, that question is still genuinely open (Camilleri 2019).

That distinction matters more than it sounds. Most of the evidence linking a permeable gut to conditions outside the digestive tract comes from studies that found the two things together, not studies that showed one caused the other. A 2024 review walking through the gut microbiota, permeability, and inflammation connection made the same point. The biology is plausible and interesting, and the human proof is still thin (Di Vincenzo 2024).

So "leaky gut" as a described physical process is reasonable. "Leaky gut syndrome" as a standalone diagnosis that explains your fatigue, your mood, and your weight is not something the evidence currently backs.

About those tests

This is the part worth saving you money on. If you have seen an at-home leaky gut panel, it is most likely measuring a blood protein called zonulin.

Researchers who actually study the gut barrier do not use those. In research settings, permeability is typically measured by having someone drink a specific sugar solution and then measuring what turns up in their urine, which shows how much actually crossed the barrier. Getting a reliable read is difficult even in a controlled study, and there is still no agreement on a single best test (Camilleri 2019).

A direct-to-consumer blood test cannot tell you whether your gut is "leaky," and it certainly cannot tell you that it is causing your symptoms. If your symptoms are real and ongoing, the useful move is a doctor who can rule out celiac disease, inflammatory bowel disease, and other conditions that have actual tests and actual treatments.

What does seem to loosen the barrier

A few things do reliably increase permeability, and none of them are surprising.

Regular use of NSAIDs, the family that includes ibuprofen and aspirin, is one of the most consistent. Heavy alcohol intake is another. So is hard endurance exercise in the heat, which is why this comes up in marathon and triathlon research (Camilleri 2019). Diet plays a role too. A detailed review of dietary components found that what you eat can shift barrier function in both directions, with fiber and a varied plant intake generally on the helpful side (Khoshbin 2020).

Notice what is not on that list: a single food you ate last Tuesday, or gluten in people who do not have celiac disease or a diagnosed sensitivity.

What actually has evidence behind it

Two supplements come up constantly in leaky gut marketing. Here is what the trials actually show.

Glutamine has the single most striking result, but read the fine print. In an eight-week randomized trial, adults who developed diarrhea-predominant IBS after a gut infection, and who had measurably increased permeability, took 5 grams of glutamine three times a day. The main symptom target was met by about 80 percent of the glutamine group compared with about 6 percent on placebo, and their permeability measurements normalized (Zhou 2019). That is a genuinely impressive result. It is also a specific group of people with a specific problem, in one trial, and the authors themselves called for larger studies before drawing firm conclusions. It is not evidence that glutamine will fix general bloating.

Probiotics have broader but softer support. A meta-analysis pooling 26 randomized trials with about 1,900 people found that probiotics improved several markers of barrier function and lowered some inflammation markers. The authors were clear that higher-quality trials are still needed (Zheng 2023). Worth trying if you are curious, not worth building a routine around.

What I would actually do

If you strip away the products, the sensible advice ends up looking boring, which is usually a good sign.

Eat a wider variety of plants and get your fiber up, since that is where the food evidence points. Go easy on routine NSAID use, and talk to your doctor if you are reaching for ibuprofen most days, because that is worth solving at the source. Keep alcohol moderate. Sleep and stress are worth attention too, given how closely the gut and nervous system are wired together. And skip the at-home permeability test.

If your symptoms are persistent, that is the signal to get evaluated rather than to self-treat. Ongoing bloating, diarrhea, or pain deserves a real workup, not a powder.

The bottom line

Your gut barrier is real, it can become more permeable, and researchers take that seriously. What has not been shown is that a leaky barrier is the hidden cause of unrelated symptoms, or that you can buy a test to confirm it, or that a supplement will seal it shut. The honest position is that this is an active area of research with more open questions than answers, and the people selling you certainty are ahead of the evidence.

Is leaky gut a real condition?

Partly. Increased intestinal permeability is a real, measurable thing, and researchers study it seriously. It shows up in conditions like celiac disease, Crohn's disease, and some cases of IBS. What is not established is leaky gut as a standalone diagnosis that causes unrelated problems like fatigue, brain fog, or weight gain. In most conditions researchers still cannot say whether the loosened barrier causes the disease or results from it.

Are at-home leaky gut tests worth buying?

No. Most direct-to-consumer panels measure a blood protein called zonulin, which is not how researchers assess the gut barrier.
